Who is sponsoring this study?
What is the drug and/or device being studied?
Who reviewed and approved this study?
What could happen to my health if I participate?
How long does the study last?
Do I need to fast?
How often are study visits?
Will I be compensated?
What kind of procedures are part of this study and what are their risks?
Is my information kept confidential?
